The LPN position is directly related to and requires assignment or delegation by and performance under the supervision, orders, or directions of the Nursing Director (RN). Individual assignments may consist of traveling between multiple clients and/or assigned to specific clients.
Including, but not limited to:
Skilled Nursing care in the clients home
Conduct re-assessment visits for continued home health services
Regularly evaluate participants nursing needs
Identification of nursing interventions and goals for review by the RN
Participates in decision-making regarding the implementation of nursing and medical interventions utilizing assessment data
Participation in multidisciplinary planning by providing resource data
Prepare clinical progress notes
Coordinate client services
Must possess and maintain a valid Licensed Practical Nurse (LPN) license in good standing in the state of Virginia
The Community Support Case manager must, at a minimum, meet the qualifications of a qualified Mental Health Professional (QMHP) with a minimum of a Bachelors Degree. At least two years of experience providing direct services to persons with serious mental illness and serious emotional disturbance. The primary consideration is the applicant’s ability to engage in a positive and professional relationship with the client while demonstrating commitment and concern to the client and services.
Including, but not limited to:
Assessment of the individual’s needs that can be met by the required activities of Mental Health Support Case Manager
Provide training and reinforcement of activities of daily living skills to clients with serious mental illness and/or dually diagnosed
Provide reinforcement of functional skills and appropriate behavior related to the individual’s health and safety, activities of daily living, and use of community resources; assistance with medication management, and monitoring health, nutrition, and physical condition
Maintain professionalism while demonstrating concern and advocacy for clients
Document each service visit on a progress note in a timely manner
Attend weekly staff meetings with Program Manager to discuss caseload and client’s progress with goal
Development of an Individualized Service Plan based on the problems and needs identified during the assessment and agreed upon by the provider and individual.
Review of the individualized service plan with the individual at least quarterly. Ongoing revisions of the ISP as treatment needs change will serve to keep the ISPs current and a complete review and revision will be completed at least annually
Assessing and coordination of community resources
Knowledge of the principles, techniques, and practices related to the provision of community-based mental health residential support services
Knowledge of casework documentation and requirements to assure compliance with regulatory requirements
Knowledge of mental illness and therapeutic interventions
Knowledge of community resources and entitlement benefits
Ability to develop and implement appropriate individual service plans
Ability to interact with referral sources and other community service providers
Ability to utilize clinical supervision to improve service delivery and documentation
